{"text": "day and World kindness day is celebrated around the world on this particular date", "start": 320.56, "end": 326.56}, {"text": "first introduced in 1998 by the world kindness mve movement World kindness Day", "start": 326.56, "end": 331.72}, {"text": "offers an opportunity to highlight good deeds in the community and the common", "start": 331.72, "end": 337.199}, {"text": "thread of kindness the day also encourages people to move away from self-centeredness and", "start": 337.199, "end": 344.479}, {"text": "focus on Collective well-being proving that compassion and kindness are", exciting day the official opening of of Amazon um operation in Richmond", "start": 395.84, "end": 403.56}, {"text": "Hill it's you know four years in the making it was you know in 2020 uh they", acknowledged our our city staff here as being extremely", "start": 416.199, "end": 421.319}, {"text": "cooperative and uh with 19 19 of these massive distribution centers in Ontario", "start": 421.319, "end": 427.199}, {"text": "and over 30 in Canada um they said that Richmond Hill was the the most uh the best city to to work with as they went", "start": 427.199, "end": 434.56}, {"text": "through the the building process for this building so um I just I just wanted", "start": 434.56, "end": 439.879}, {"text": "to acknowledge that thank you Mr Mayor thank you very much that's well said {"text": "different aspects of Richmond Hill including the Dunlop Observatory um you know even down to Monarch butterflies", "start": 504.879, "end": 511.72}, {"text": "with the fact that we're a a mayor's Monarch pledge signatory and all kinds of landmarks and and aspects of the uh", "start": 511.72, "end": 518.64}, {"text": "Community including the Jefferson salamander which was uh you know there", "start": 518.64, "end": 523.76}, {"text": "as part of the mural as well so they've really done their homework on Richmond Hill and I thought that was very gratifying um I also would also like to", "start": 523.76, "end": 531.24}, {"text": "uh just put out a a shout out to the Royal Canadian Legion Branch 375 uh and our staff who helped to", "start": 531.24, "end": 538.6}, {"text": "organize that event um the Remembrance Day event both on Sunday uh and on", "start": 538.6, "end": 544}, {"text": "Monday and uh you know they do a great job and have been doing a great job for many here", "start": 748.12, "end": 754.44}, {"text": "today um we we and Regional Council get presentations from the paramedics on a regular basis but there's some exciting", "start": 754.44, "end": 761.48}, {"text": "stuff and important things happening with Paramedic Services uh in all of of", "start": 761.48, "end": 767.279}, {"text": "the region of York and I I really am pleased to uh invite the privilege of being the uh currently the acting chief of our York Region uh paramedic service and uh", "start": 786.639, "end": 793.16}, {"text": "thank you for the opportunity uh to be able to present an update on the roll out of the new medical priority dispatch", "start": 793.16, "end": 798.48}, {"text": "system known as mpds I'll be providing an overiew an overview of the expected", "start": 798.48, "end": 803.639}, {"text": "changes and the impacts on your residence um as requested by Mayor West today", "start": 803.639, "end": 810.36}, {"text": "so today's today's presentation I I also hope to cover um an overview of York Region Paramedic Services including", "start": 811.959, "end": 817.92}, {"text": "response time information um very much um around uh Richmond Hill the need uh", "start": 817.92, "end": 823.68}, {"text": "for dispatch modernization and why the current 911 ambulance uh dispatch system", "start": 823.68, "end": 828.76}, {"text": "requires these updates uh progress on the implementation of M mpds at the", "start": 828.76, "end": 834.199}, {"text": "Georgian Central ambulance Communication Center or CAC and spoiler alert we actually to go live today so we're", "start": 834.199, "end": 840.639}, {"text": "really excited we're really excited for that it's been a long time uh coming uh I also hope to cover what mpds means for", "start": 840.639, "end": 846.199}, {"text": "our services and and potential future benefits um of the provincial government initiated dispatch modernization project", "start": 846.199, "end": 853.04}, {"text": "and mpds is one of the final components of this modernization effort and is crucial for future planning of Paramedic", "start": 853.04, "end": 858.56}, {"text": "Services across the province we look forward to discussing how these advancements will enhance our emergency", "start": 858.56, "end": 864}, {"text": "response capabilities and improve Service delivery within uh our community", "start": 864, "end": 870.279}, {"text": "uh first I wanted to provide an overview of our of our service so York Region Paramedic Services proudly provides", "start": 870.839, "end": 876.16}, {"text": "Round the Clock emergency and Non-Emergency Medical Response across all municipalities within New York", "start": 876.16, "end": 881.44}, {"text": "Region in 2023 our paramedics responded to approximately 93,000 incidents beyond", "start": 881.44, "end": 889.079}, {"text": "the emergency response capacity we also support our community Through Innovative Community paramedicine programs and new", "start": 889.079, "end": 895.16}, {"text": "models of care providing Continuous Care for those most in need our strength lies within our people with", "start": 895.16, "end": 902.16}, {"text": "over 600 plus full-time staff including 530 Frontline paramedics we're always", "start": 902.16, "end": 907.72}, {"text": "ready to respond York Region is rapidly growing and diversifying we anticipate a", "start": 907.72, "end": 914.199}, {"text": "6.8% annual increase in call demand through 2031 driven by Iran expansion in", "start": 914.199, "end": 920.12}, {"text": "areas like Richmond Hill Markham and vaugh and also significant growth uh seen um in more rural areas uh currently", "start": 920.12, "end": 927.24}, {"text": "in in East gbury our aging population uh presents unique", "start": 927.24, "end": 932.8}, {"text": "challenges by 2051 seniors will make up 62% of our population which will be up", "start": 932.8, "end": 938.68}, {"text": "significantly from the 42% that it represented in 2021 seniors over 75 are twice as likely", "start": 938.68, "end": 945.399}, {"text": "to require hospitalization or engage Paramedic Services compared to those aged 55 to", "start": 945.399, "end": 951.759}, {"text": "74 and in 2023 58% of our 911 calls came", "start": 951.759, "end": 957.04}, {"text": "from Seniors age 65 and over older highlighting the critical role that paramedics play in their journey of", "start": 957.04, "end": 965.6}, {"text": "care so before I I take you through some of the 911 system uh changes I wanted to provide an overview of the current uh", "start": 967.24, "end": 973.44}, {"text": "paramedic response landscape uh within Richmond Hill so Richmond Hill currently has three existing stations that are", "start": 973.44, "end": 979.88}, {"text": "directly supporting it with four surrounding uh stations uh providing additional uh response capacity as part", "start": 979.88, "end": 986.839}, {"text": "of our master plan two new stations are proposed in the surrounding vicinity to enhance the overall uh coverage within", "start": 986.839, "end": 993.44}, {"text": "within the area uh yearly 911 call volume saw a slight increase over previous years and", "start": 993.44, "end": 1000.079}, {"text": "for the year of 2023 uh we responded to 14,32 calls just within Richmond Hill", "start": 1000.079, "end": 1006.16}, {"text": "alone and I wanted to highlight uh sudden Cardiac Arrest um response time", "start": 1006.16, "end": 1011.759}, {"text": "metrics which is a key response time Target that is set by The Province um these are are are", "start": 1011.759, "end": 1018.079}, {"text": "categorized as our most um uh Urgent Response and the The Province sets a target of um arriving within six uh 6", "start": 1018.079, "end": 1025.798}, {"text": "minutes at least 60% of the time in Richmond Hills response time for sudden Cardiac Arrest was met um 75% of the", "start": 1025.799, "end": 1033.52}, {"text": "time and 68% of the time in 2022 and 2023 um as well I just wanted to", "start": 1033.52, "end": 1039.48}, {"text": "highlight Richmond Hill is is more urban than some of the surrounding areas so response times are generally better compared to more rural areas but can be", "start": 1039.48, "end": 1046.839}, {"text": "uh significantly impacted by factors such as traffic congestion as well as vertical response which for us means", "start": 1046.839, "end": 1051.96}, {"text": "when we respond to uh higher levels of of of buildings it also it takes a longer time to connect with those uh", "start": 1051.96, "end": 1057.84}, {"text": "folks that are calling uh we have an excellent uh partnership with McKenzie Health Richmond Hill uh who's one of the", "start": 1057.84, "end": 1063.96}, {"text": "the better performing hospitals in terms of ambulance offload delay not only within the region but to be honest within the within the province this", "start": 1063.96, "end": 1070.84}, {"text": "supports our our paramedics returning to the community faster um as they were able to transfer care of their previous", "start": 1070.84, "end": 1076.72}, {"text": "P patient and return to the community which increases our response capacity which is uh which is definitely um most", "start": 1076.72, "end": 1082.96}, {"text": "areas of the province would love to uh see the the the targets that uh uh Mackenzie Health Richmond Hill is able", "start": 1082.96, "end": 1088.64}, {"text": "to maintain um just jumping back to um the", "start": 1088.64, "end": 1093.919}, {"text": "I wanted to provide an overview of sort of how the current 911 system works in in the call taking workflow so 911 calls", "start": 1093.919, "end": 1100.84}, {"text": "in York Region are initially answered by York Regional Police through what is known as the public safety answering", "start": 1100.84, "end": 1106.2}, {"text": "point which is also known as a peap York Regional Police transfers these these calls to secondary P Ops which include", "start": 1106.2, "end": 1113.2}, {"text": "Richmond Hill fire and emergency services and the Georgian Central ambulance Communication Center or the", "start": 1113.2, "end": 1118.72}, {"text": "CAC of note y RP's p app is transitioning to the Next Generation 911", "start": 1118.72, "end": 1124.28}, {"text": "Network which will allow the system to better triangulate a person's location which will improve the accuracy of", "start": 1124.28, "end": 1129.679}, {"text": "identifying the call location YP then uh once it transfers the 91 call to the Georgian CAC the", "start": 1129.679, "end": 1137.28}, {"text": "ambulance dispatcher uh prioritizes the call which is currently based on a two priority response system once", "start": 1137.28, "end": 1143.72}, {"text": "prioritized the closest available ambulance is notified and then paramedics respond to the emergency the", "start": 1143.72, "end": 1148.799}, {"text": "response time uh clock starts once the paramedic receives notification and once paramedics arrive at the scene they", "start": 1148.799, "end": 1155.2}, {"text": "assess and either transport the patient to hospital or they may treat and refer or release to scene reducing the need", "start": 1155.2, "end": 1161.08}, {"text": "for transport to the hospital now I'd like to play a short video that demonstrates the call Process under the", "start": 1161.08, "end": 1166.52}, {"text": "new mpds system and has been integrated within our um with within our public uh", "start": 1166.52, "end": 1174.919}, {"text": "engagement York Region Paramedic Services are there when our community needs them the most we provide emergency", "start": 1182.12, "end": 1189}, {"text": "and Non-Emergency Medical Response 24 hours a day 7 days a week our skilled", "start": 1189, "end": 1194.679}, {"text": "smart and strong team includes Frontline paramedics specialty teams and support staff all dedicated to delivering the", "start": 1194.679, "end": 1201.799}, {"text": "highest quality of care in November of 2024 the medical priority dispatch", "start": 1201.799, "end": 1207.48}, {"text": "system or mpds will be implemented as part of the Ontario government's Your Health Plan introduced by the Ministry", "start": 1207.48, "end": 1214.559}, {"text": "of Health this evidence-based technology used globally will transform how 911", "start": 1214.559, "end": 1220.08}, {"text": "calls are prioritized in York Region mpds ensures that emergency calls are prioritized based on urgency this means", "start": 1220.08, "end": 1227.679}, {"text": "patients will receive care when they need it most life-threatening cases are given top priority ensuring the most", "start": 1227.679, "end": 1233.96}, {"text": "timely and appropriate level of outof hospital care is available for all in our community when anyone calls 911", "start": 1233.96, "end": 1241.08}, {"text": "Communications officers will ask detailed questions to gather specific information about the patient's", "start": 1241.08, "end": 1246.44}, {"text": "condition ambulance what is the address of your emergency what's the phone number including area code that you're calling", "start": 1246.44, "end": 1252.88}, {"text": "from are you with the patient now okay tell me exactly what happened is there", "start": 1252.88, "end": 1258.4}, {"text": "any any serious bleeding has he ever had a heart attack or angina this allows for quick and effective incident", "start": 1258.4, "end": 1264.799}, {"text": "prioritization sending paramedics to the most urgent cases first but ensuring all of our community has access to care", "start": 1264.799, "end": 1271.24}, {"text": "through the 911 system lights and Sirens are used to respond to and transport patients with the most urgent needs such", "start": 1271.24, "end": 1278.279}, {"text": "as a heart attack stroke or a serious traumatic injury ensuring prompt transfer to the hospital under our", "start": 1278.279, "end": 1284.6}, {"text": "paramedics highly skilled care when necessary for non-life-threatening I conditions such as sprains or broken", "start": 1284.6, "end": 1290.44}, {"text": "bones there may be a longer wait time Communications officers will continue to monitor the patient status with", "start": 1290.44, "end": 1296.559}, {"text": "follow-up calls hi there this is the ambulance service calling back we have been delayed has there been any change", "start": 1296.559, "end": 1302.679}, {"text": "to the patient's condition or circumstance if their conditions worsen the call will be rep prioritized and", "start": 1302.679, "end": 1309.6}, {"text": "help will be sent without delay it's important to remember that a longer wait time doesn't mean your call is not", "start": 1309.6, "end": 1314.64}, {"text": "important we understand how stressful it can be rest assured York Region paramedics are committed to helping you", "start": 1314.64, "end": 1321.12}, {"text": "as quickly as possible the health and well-being of our residents is our top priority our paramedics are dedicated to", "start": 1321.12, "end": 1328.159}, {"text": "delivering care and saving lives for more information on the medical priority dispatch system visit", "start": 1328.159, "end": 1334.559}, {"text": "york. Paramedic Services thank you as you saw in the", "start": 1334.559, "end": 1341.6}, {"text": "video um mpds replaces the old system expanding emergency response levels from", "start": 1341.6, "end": 1346.76}, {"text": "2 to 5 this allows for more more accurate prioritization and resource allocation I want to highlight what this", "start": 1346.76, "end": 1353.279}, {"text": "may mean for you and your role as elected officials within our community you may receive calls from constituents", "start": 1353.279, "end": 1358.88}, {"text": "if there are delays in ambulance response times for Less urgent calls this is a common experience in other regions that have transitioned already", "start": 1358.88, "end": 1365.279}, {"text": "to this mpds system there are key messages we want you and the response time targets the master plan is a strategic", "start": 1590.52, "end": 1597.96}, {"text": "road map so that York Region Paramedic Services can continue to meet growing demands and maintain a "walk-in clinic do do you mean that it will be determined that not every single called needs an", "start": 1798.919, "end": 1804.88}, {"text": "ambulance uh thank you uh so much for the question counselor and and uh through you uh uh chair um so those are", "start": 1804.88, "end": 1813.24}, {"text": "um the mpds is the first step so that's currently not uh part of it but what it will mean what's that mean sorry mpds", "start": 1813.24, "end": 1820.84}, {"text": "mpds so the the new um medical priority dispatch system that we're changing to", "start": 1820.84, "end": 1826.2}, {"text": "um so the uh the caller um not having an", "start": 1826.2, "end": 1831.519}, {"text": "ambulance sent or being referred directly to an alternate care pathway is not part of the current roll out but the", "start": 1831.519, "end": 1838.159}, {"text": "new dispatch system being mpds will allow us to be able to implement some of", "start": 1838.159, "end": 1843.2}, {"text": "those things in the end of the future so what it will mean uh currently for residents if they have a low Acuity so", "start": 1843.2, "end": 1849.24}, {"text": "for example if someone calls and says I have a runny nose um and for whatever reason they've activated 911 They will", "start": 1849.24, "end": 1855.919}, {"text": "receive an ambulance but it is certainly not going to be um our quickest response so once the system has an availability", "start": 1855.919, "end": 1863.12}, {"text": "to to to do that call they will in the in the future having this new system um", "start": 1863.12, "end": 1868.639}, {"text": "allows us to um uh instead of it being one or two response priorities it actually opens it up to thousands to", "start": 1868.639, "end": 1874.72}, {"text": "prioritize the calls and what that means for us in in the in the future and uh is being able to try uh new and Innovative", "start": 1874.72, "end": 1882.44}, {"text": "programs so for example actually having healthcare providers in the dispatch system having um an actual interaction", "start": 1882.44, "end": 1888.679}, {"text": "with a 911 caller and in the future it opens the opportunity uh that they may not receive an ambulance but will", "start": 1888.679, "end": 1894.679}, {"text": "receive a better referral to a better uh more appropriate Healthcare Resource thank you through you Mr chair", "start": 1894.679, "end": 1901.279}, {"text": "um so the part about you won't be running sirens is alarming to people", "start": 1901.279, "end": 1906.72}, {"text": "like me what that feels like less urgency when people are supposed to call", "start": 1906.72, "end": 1912.96}, {"text": "an amulance when there's urgency excellent question and so we current currently uh we currently run uh", "start": 1912.96, "end": 1920.519}, {"text": "70% of the time uh lights and siren response with mpds uh as it uh is a very", "start": 1920.519, "end": 1927.039}, {"text": "much a medically validated tool it is better to Able understand what uh the um", "start": 1927.039, "end": 1933.2}, {"text": "uh the the issues and the the clinical condition of the individual and um we", "start": 1933.2, "end": 1938.48}, {"text": "will still certainly be running lights and siren to uh high level calls but we expect that number to be similar to", "start": 1938.48, "end": 1944.48}, {"text": "other uh uh regions that have already moved to this and in from 70% of the time lights and siren to 50% of the time", "start": 1944.48, "end": 1951.399}, {"text": "and what it means is the that 20% difference um of of that case load it", "start": 1951.399, "end": 1956.679}, {"text": "doesn't mean that we're not having an Urgent Response it means that the uh risks of driving lights and siren um", "start": 1956.679, "end": 1962.919}, {"text": "through congested traffic areas uh through um intersections it it's the the", "start": 1962.919, "end": 1968.679}, {"text": "uh risk benefit ratio isn't isn't there it doesn't mean that for someone say who's having chest pain or who's not", "start": 1968.679, "end": 1975.24}, {"text": "breathing certainly lights and siren most Urgent Response that we still expect to be 50% of our our cases um but", "start": 1975.24, "end": 1981.32}, {"text": "there's a a lot of research that indicates the use of lights and siren and the impact that it has on traffic safety for the providers and for the", "start": 1981.32, "end": 1988.279}, {"text": "public and so it's an opportunity to improve um uh Road Safety as well so it's really a balance of all those", "start": 1988.279, "end": 1994.44}, {"text": "things thank you three Mr chair um is the video we saw for the general public", "start": 1994.44, "end": 2000.12}, {"text": "or for Council uh uh thank you uh that is uh that will be the video that will", "start": 2000.12, "end": 2005.44}, {"text": "be for the the the general uh count uh uh for Council but also we're going to be using uh the same video for um public", "start": impression that if my kid falls at the park I should call 911 when I should throw the kid in the car and go to the local Clinic it I when I saw that video", "start": 2019.48, "end": 2027.679}, able to respond through through you chair um for for that comment um that is the reality of of the", "start": 2034.399, "end": 2040.08}, {"text": "of the system and we we do see in that as the the reason for having better um a", "start": 2040.08, "end": 2045.159}, {"text": "better clinically validated tool to ask these questions so to to determine the right response uh to that so um", "start": "really good questions U at the intake phase and to really understand the patient condition and to provide a", "start": 2110.24, "end": 2116.44}, {"text": "response that uh reflects the urgency of of the matter and this is a system that's uh been used around the world and", "start": 2116.44, "end": 2122.92}, {"text": "and it's it's it's very clinically safe to do so and that's and that's what we're seeing okay uh Regional local", "start": 2122.92, "end": 2128.599}, {"text": "counselor depalo thank you mayor um is the as uh", "start": 2128.599, "end": 2136.04}, {"text": "people's driving behavior changed it um it sounds like a a big part of this is", "start": 2136.04, "end": 2142.599}, {"text": "to um you know have less ambulances with lights and Sirens uh um is is is it", "start": 2142.599, "end": 2151.359}, {"text": "seems that uh people aren't as respectful of ambulances as they they should be or used to be uh", "start": 2151.359, "end": 2158.319}, {"text": "you know if everyone was pulling over and clearing the way is the data showing some uh an uptick in in accidents caused", "start": 2158.319, "end": 2165.4}, {"text": "by emergency vehicles uh so thanks so much for the question and and through the chair um", "start": 2165.4, "end": 2171.8}, {"text": "the as I mentioned there are there is research that indicates the use of Warning Systems in emergency settings", "start": 2171.8, "end": 2177.88}, {"text": "across North America doesn't increase um the likelihood of um traffic incidents", "start": 2177.88, "end": 2183.4}, {"text": "um that is while that is um uh decreasing the lights and IR response sort of percentages is a byproduct", "start": 2183.4, "end": 2190.76}, {"text": "really what it is is that it's it allows us to better prioritize so um to put it", "start": 2190.76, "end": 2195.92}, {"text": "another way if uh 70% of the cases are the highest emergency it doesn't leave", "start": 2195.92, "end": 2201.96}, {"text": "much uh room to sort them and so for example you could have someone that is", "start": 2201.96, "end": 2207.359}, {"text": "having a chest pain um and we get there and it's you know not necessarily urgent and someone that's stopped breathing in", "start": 2207.359, "end": 2213.8}, {"text": "the current system whichever one comes in first is what's getting the getting the um the closest ambulance in the", "start": 2213.8, "end": 2219.68}, {"text": "current system it will look at both of those it will even if the say the person who's um having um the the chest pain", "start": 2219.68, "end": 2226.2}, {"text": "versus someone not breathing um it will send the vehicle to the person having chest pain but then if a higher priority", "start": 2226.2, "end": 2232.68}, {"text": "calls it'll move the uh response to the highest priority call so it really it's", "start": 2232.68, "end": 2237.72}, {"text": "more around how do we prioritize the response and how do we provide an appropriate response to the level of", "start": 2237.72, "end": 2244.04}, {"text": "urgency that we're seeing in the in the in the call so while the byproduct is less lights and sirm responses it's um", "start": 2244.04, "end": 2251.72}, {"text": "it's not the immediate Focus it's a uh certainly a way to ensure that we have the best uh uh safest response capacity", "start": 2251.72, "end": 2260.04}, {"text": "and that's um these talk about maybe a reduction or balance", "start": 2307.92, "end": 2313.359}, {"text": "use um when appropriate would that mean maybe um certain it at busy hours on the", "start": 2313.359, "end": 2322.319}, {"text": "road or maybe in a u um more highly dense area the use of", "start": 2322.319, "end": 2330.24}, {"text": "sirens might have different flexible guidelines uh for the", "start": 2330.24, "end": 2335.359}, {"text": "driver or for the paramedic to uh to make decision on whether it's Now to S", "start": 2335.359, "end": 2341}, {"text": "it or now not to song it is there any flexibility period um thank thank you", "start": 2341, "end": 2347.04}, {"text": "for the thank you for the question uh and through the through the through the chair um the like I said the the", "start": 2347.04, "end": 2352.92}, {"text": "decrease in the lights and siren response is an outcome of just understanding um a better understanding", "start": 2352.92, "end": 2360.079}, {"text": "the urgency of the situation better um so what it means is um currently we", "start": 2360.079, "end": 2365.319}, {"text": "would go lights and siren but it was a non-urgent case meaning that you know it was a case that we didn't have to rush", "start": 2365.319, "end": 2372.2}, {"text": "there very quickly um and what that does is twofold one um it increases uh sort", "start": 2372.2, "end": 2378.2}, {"text": "of risks around like I mentioned uh traffic incidents but uh secondly it just means that that vehicle isn't", "start": 2378.2, "end": 2384.96}, {"text": "necessarily available for uh like a truly highly urgent case and that's really what the design of the system is", "start": 2384.96, "end": 2391.48}, {"text": "is is really more around efficiency and really um uh understanding what the 911", "start": 2391.48, "end": 2396.72}, {"text": "calls uh t uh coming in the urgency level is and just providing response that meets that level instead of just", "start": 2396.72, "end": 2403.04}, {"text": "responding with lights and siren to everything which um we do very well in New York Region uh but certainly across", "start": 2403.04, "end": 2409}, {"text": "Ontario and in Canada and even internationally um you'll certainly see you know um media cases where ambulances", "start": 2409, "end": 2415.2}, {"text": "weren't available for Urgent cases we haven't had anything like that it's it is because of the support of uh councils", "start": 2415.2, "end": 2421.8}, {"text": "and it is through the support of um really investing in our system and our infrastructure uh through our master", "start": 2421.8, "end": 2427.359}, {"text": "planning process but this will allow us to really make sure that we will always have um the right response uh um", "start": 2427.359, "end": 2434.52}, {"text": "capacity that's available um for the most urgent of cases and that's that's obviously the biggest risk for us is not", "start": 2434.52, "end": 2439.839}, {"text": "having an ambulance for um someone who's really urgent thank you for the answer", "start": 2439.839, "end": 2445.28}, {"text": "iting two wat so if I understand correctly the factors for considerations um not so much the ongoing assessment by the", "start": 2506.119, "end": 2512.16}, {"text": "by the paramics fore Mr chair yeah yes thank thank you thank you through for the for the question question counselor", {"text": "um uh a challenge to find um uh folks with the right education and and", "start": 2559.2, "end": 2565.839}, {"text": "understanding that there is a sort of a bit bit of a a deficit from while we um were completing the uh the build of our station in Thornhill on Glen Cameron which is a", "start": 2628.68, "end": 2635.48}, {"text": "little bit south of there uh since we've uh been collocated with them there's", "start": 2635.48, "end": 2640.68}, {"text": "been a lot of work that's happened at the Province and certainly you you would be very um privy to the population", "start": 2640.68, "end": 2647.52}, {"text": "Targets in that area around Highway 7 and and Young and the and the transit Corridor uh so where we're currently at", "start": 2647.52, "end": 2653.839}, {"text": "with that we're in the preliminary stages of um in working with Chief Burbage to really uh look at um the", "start": 2653.839, "end": 2660.28}, {"text": "opportunities whether to to collocate but I think both of the services really need to understand um the future uh", "start": 2660.28, "end": 2666.28}, {"text": "demand needs and in that area and we hope to be able to bring something back to replacing which is in in Maple on the West Side um and then we're", "start": 2894.599, "end": 2901.119}, {"text": "also looking at expanding um uh sort of what's in Unionville and so um it really is around um how you uh set up for a", "start": 2901.119, "end": 2909.64}, {"text": "network the vehicles that are available and then they also it's very that we move them around the station and so for", health call and how well", "start": 2971.44, "end": 2979.119}, {"text": "equipped our par paramedics to deal with mental health or will rely on the police", "start": 2979.119, "end": 2984.96}, {"text": "through you Mr Mayor uh thank thank you for the the counselor and and through the mayor um yes we I don't have the the", "start": 2984.96, "end": 2991.119}, {"text": "figures but we certainly see um increasing responses for um uh calls", "start": 2991.119, "end": 2996.28}, {"text": "that are related to mental health um we work very closely with uh York Regional", "start": 2996.28, "end": 3001.48}, {"text": "Police um but it's broader like the issue in in the solution is certainly broader it's um integrating all this", "start": 3001.48, "end": 3008.599}, {"text": "work that's that's happening um with the health care System locally there's a lot of work that's happening with the", "start": 3008.599, "end": 3014.2}, {"text": "development of a mental health uh Hub that's going to be you know in in New Market um and and there's a lot of work", "start": 3014.2, "end": 3021.04}, {"text": "it it certainly is broader we do a lot of work uh with the with the paramedics in terms of increasing their", "start": 3021.04, "end": 3027.2}, {"text": "competency around managing um uh uh incidences that involve Mental Health", "start": 3027.2, "end": 3032.88}, {"text": "crisis working very closely with the with the police um but as an offshoot it is uh it is very difficult when you have", "start": 3032.88, "end": 3040.44}, {"text": "someone in crisis in an emergent situation and that's why more infrastructure and more access to", "start": 3040.44, "end": 3046}, {"text": "inpatient and outpatient services is required and we certainly have a long way to go but there's some really good", "start": 3046, "end": 3051.44}, {"text": "initial steps that are that are occurring with I think the right people at the the table in my opinion but absolutely it's it's a great question", "start": 3051.44, "end": 3058}, {"text": "thank you um as we can see at least in Richmond Hill um there's a lots of", "start": 3058, "end": 3063.4}, {"text": "proposal about long-term cares and and and senior homes senior apartments um", "start": 3063.4, "end": 3070.72}, {"text": "and I can see there might be a higher demand for paramedics um are we prepared", "start": 3070.72, "end": 3076.68}, {"text": "for that great uh great great question and and uh for uh through you Mr Mayor", "start": 3076.68, "end": 3081.92}, {"text": "um as as I mentioned in the um in the presentation um as as age uh our chances", "start": 3081.92, "end": 3088.4}, {"text": "of hospitalization and in accessing paramedic service it it like it's exceptionally high and so that is", "start": 3088.4, "end": 3094.559}, {"text": "factored into the work we do with our planning and our demand so we look at essentially uh population like just pure", "start": 3094.559, "end": 3101.44}, {"text": "population growth we look at uh the age of the population and uh the General Health of the population to help us", "start": 3101.44, "end": 3107.559}, {"text": "project and we also look at um um another factor which is really around consumption rates of our service because", "start": 3107.559, "end": 3113.72}, {"text": "we can't really control who calls 911 so we just they call and we respond and so", "start": 3113.72, "end": 3119.96}, {"text": "one of the things that you see it's it's multifactoral in that uh in the in the um in the uh uh accessing 911 service so", "start": 3119.96, "end": 3127.599}, {"text": "for example if folks don't have access to primary care if they don't have access to care in the community then", "start": 3127.599, "end": 3133.64}, {"text": "they they call 911 or they access through the emergency department so those are all the those are all the the", "start": 3133.64, "end": 3139.04}, {"text": "things but certainly um uh uh Care for Seniors is um one of the things that is", "start": 3139.04, "end": 3144.599}, {"text": "a big driver for our um and and not just enhancing it but there's a ton of work we're doing on our community", "start": 3144.599, "end": 3150.599}, {"text": "paramedicine side which I briefly mentioned and a lot of what that is is it's uh new models of care that uh we're", "start": 3150.599, "end": 3156.92}, {"text": "involved in a number of uh Pilots that are really looking at how do you best support um Aging in place so one of them", "start": 3156.92, "end": 3163.24}, {"text": "is our IMPACT Program which is uh a new model of care approved by the the province of Ontario where we've trained", "start": 3163.24, "end": 3169.119}, {"text": "uh currently a small number of paramedics but um we're expanding that number in uh how to function in the", "start": 3169.119, "end": 3174.799}, {"text": "primary care environment and right now you know they'll receive a 911 call they may assess the patient they may do point", "start": 3174.799, "end": 3180.44}, {"text": "of care blood work they may do year analysis they will work with a physici and the outcome is often that they're", "start": 3180.44, "end": 3186.64}, {"text": "actually discharging these these these folks at home and so a lot of this cohort is seniors we have a number of uh", "start": 3186.64, "end": 3192.599}, {"text": "models of care that are actually targeting one just launched last week it's called our uh cptc plus which is", "start": 3192.599, "end": 3198.119}, {"text": "community par paramedicine in the long-term care home plus and what that is is partnering with various um nursing", "start": 3198.119, "end": 3203.96}, {"text": "homes um and their uh uh medic staff when um a lot of these residents um of", "start": 3203.96, "end": 3209.92}, {"text": "these long-term care homes are being transferred to the hospital for Diagnostic uh issues when uh they're not", "start": 3209.92, "end": 3216}, {"text": "really receiving that so paramedics are being special specially trained to do point of care blood work to do point of", "start": 3216, "end": 3221.96}, {"text": "care ultrasound and a number of these other factors and these are uh small pilots and we're really looking at sort", "start": 3221.96, "end": 3228.119}, {"text": "of the evaluation of them and expanding them so it's it's not just around expanding our Workforce and continuing to just add ambulances it's how do we", "start": 3228.119, "end": 3235.319}, {"text": "provide the right care at the right time in the right place and it's and just increasing the infrastructure within our", "start": 3235.319, "end": 3240.52}, {"text": "system but excellent question and obviously I could talk forever about it so very very passionate about it languages so it's um that's a", "start": 3257.88, "end": 3264.72}, {"text": "that's a fantastic uh question in through you chair U it is definitely an area where we saw as an opportunity um", "start": 3264.72, "end": 3271.28}, {"text": "10 years ago our Workforce was uh I would say not very diverse um uh we did", "start": 3271.28, "end": 3276.72}, {"text": "not have a number of our our folks that spoke uh different languages and that has been um really gradually gradually", "start": 3276.72, "end": 3283.4}, {"text": "changing and we're really doing a lot of work in the EDI space internally um to not only make um uh ours A Premier", "start": 3283.4, "end": 3291.2}, {"text": "Service to come and work at but that they have the supports that they feel supported in having a diverse Workforce", "start": 3291.2, "end": 3296.64}, {"text": "that is reflective of the community is is like is is really our goal and we're certainly seeing um every Rec recruit", "start": 3296.64, "end": 3302.96}, {"text": "class uh more diverse in uh the the cultural backgrounds and the languages that we are speaking um and we have", "start": 3302.96, "end": 3309.44}, {"text": "certainly had some targeted um The Challenge we have in the paramedic service is um because of the legislation", "start": 3309.44, "end": 3316}, {"text": "uh you have to decide you want to be a paramedic you go and you do um schooling and a post-secondary education and", "start": 3316, "end": 3322.2}, {"text": "that's the pool we can pull from so unlike the uh fire or police where certainly their post-secondary", "start": 3322.2, "end": 3328.599}, {"text": "definitely comes into it it's a legislative requirement so it's really around Grassroots work uh trying to", "start": 3328.599, "end": 3333.96}, {"text": "engage the community um we had a uh you know an opportunity even this week I was at a number of Remembrance Day", "start": 3333.96, "end": 3340.2}, {"text": "ceremonies and really just engaging uh Youth and and uh I had an opportunity to speak earlier this year at an event um", "start": 3340.2, "end": 3346.88}, {"text": "in um in Markham but it had a uh you know there's about a few hundred um air Cadets and navy Cadets and and it's", "start": 3346.88, "end": 3353.92}, {"text": "those those those groups and really um when we're there with them is explaining to folks that may uh speak different", "start": 3353.92, "end": 3360.64}, {"text": "languages that definitely um those are the folks that we really want to tap into and that only makes the the", "start": 3360.64, "end": 3365.799}, {"text": where I look over my balcony it certainly seems that way I mean yes they have to still stop at the the traffic", "start": 3426.119, "end": 3432.88}, {"text": "lights but they've got a free run between between lights and and to me I I", "start": 3432.88, "end": 3438.64}, {"text": "think it's fantastic can you comment on that through you Mr Mayor thank you thank you for the question counselor and", something we would really want to look at and measure", "start": 3456.48, "end": 3462.28}, {"text": "and it is certainly um we're seeing congestion in certain areas especially around rush hour it is the same things but certainly um having infrastructure like that that has open roadway is always a benefit in", "start": 3475.359, "end": 3482.48}, {"text": "a highly urbanized environment to uh to get to places quicker so I don't have it Quantified but I believe it's a really", "start": 3482.48, "end": 3488.68}, {"text": "good strategy so thank you thank you no I I def as an as an as someone from the", "start": 3488.68, "end": 3493.88}, {"text": "outside looking in I believe there's no question it is because they don't have to you know wait for people to move out", "start": 3493.88, "presentation under Staffing you you say there are 55 casual paramedics through you Mr Mayor can you explain what a", "start": 3509.559, "end": 3516.88}, {"text": "casual paramedic is sure thank you for the question and uh through you Mr Mayor so we have uh a number of", "start": 3516.88, "end": 3523.359}, {"text": "classifications so we have uh permanent full-time we have uh temporary full-time", "start": 3523.359, "end": 3528.559}, {"text": "which are um essentially uh fully certified paramedics uh generally it's an entry uh into the the system and", "start": 3528.559, "end": 3536.28}, {"text": "they're there backfilling um folks that are out of the workplace so uh being um", "start": 3536.28, "end": 3542}, {"text": "a trauma exposed profession that is very physically demanding we do see uh very high wsib numbers for um uh physical and", "start": 3542, "end": 3549.88}, {"text": "for non-physical injuries uh we also have a relatively young Workforce and so even things like um expanding um", "start": 3549.88, "end": 3556.76}, {"text": "maternity benefits to 18 months and it just increases the amount of time where we don't have staff actively in the", "start": 3556.76, "end": 3561.96}, {"text": "workplace so we use temporary full-time and what we call Casual so a casual paramedic um uh they're fully certified", "start": 3561.96, "end": 3568.799}, {"text": "as a paramedic um some of them are uh due to family circumstances so they they", "start": 3568.799, "end": 3574.96}, {"text": "are able to pick their schedule they don't work a full-time schedule uh but they're they're they're the same certification level as all of our um", "start": 3574.96, "end": 3581.96}, {"text": "other paramedics or full-time paramedics um and we also you know some of them work uh other jobs we have uh they're", "start": 3581.96, "end": 3588.119}, {"text": "full-time firefighters and they work parttime with us so it allows us to increase our uh capacity um in our in", "start": 3588.119, "end": 3595.64}, {"text": "terms of our Staffing numbers and fill in for where we have uh challenges over the summer or with our long-term leaves", "start": 3595.64, "end": 3601.28}, {"text": "and things like that so yeah it's a it's a staffing strategy that has um", "start": 3601.28, "end": 3607.88}, {"text": "uh positives and and negatives I guess for for it but uh {"text": "just sit tight and not really uh paying attention to the the change in the", "start": 3714.72, "end": 3719.96}, {"text": "condition I just wonder if you could speak to that maybe no happy to and uh through you through you Mr chair so uh", "start": 3719.96, "end": 3726.64}, {"text": "the mpds system is medically validated over millions and millions of cases it's", "start": 3726.64, "end": 3732.039}, {"text": "used worldwide it's used across North America um and it is essentially the um", "start": 3732.039, "end": 3738.44}, {"text": "uh I would I would call it the gold standard currently for prioritizing calls um there's uh it it does follow", "start": 3738.44, "end": 3745.079}, {"text": "the process but it it also does allow to have our very experienced call takers uh be able to use their own judgment and to", "start": 3745.079, "end": 3751.76}, {"text": "up upgrade calls and upgrade cases and so um while there is a theoretical risk", "start": 3751.76, "end": not yeah yes Mr Mayor so", "start": 3993.24, "end": 3999}, {"text": "that um that is correct uh we use a Dynamic Staffing model so it um starting", "start": 3999, "end": 4004.96}, {"text": "about 6 or 7 o'clock in the morning till about 10 o'clock at night is our peak time and certainly between the hours of", "start": 4004.96, "end": 4011.64}, {"text": "you know 2 to 5 it's the it's the lowest and we try and adapt our Staffing uh to", "start": 4011.64, "end": 4016.799}, {"text": "that um day shift depending on the station um certainly we have more rural", "start": 4016.799, "end": 4022.079}, {"text": "stations in the region that um they may spend more time responding just because of the population bases lower there", "start": 4022.079, "end": 4028.559}, {"text": "whereas you know within the south end of York Region on uh day shifts it's it's very common for the paramedics to leave", "start": 4028.559, "end": 4034.52}, {"text": "and to not return to a station throughout the day they essentially go from call to call and to hospital to hospital uh but um uh our design in", "start": 4034.52, "end": 4043.559}, {"text": "terms of the master plan in terms of the Capital infrastructure of stations it really is designed as a as a network and", "start": 4043.559, "end": 4050.079}, {"text": "that if that was the default that is really what we measure it from and when we work with our uh consultant they will", "start": 4050.079, "end": 4055.72}, {"text": "provide us with a it's almost like a heat map and so if we look at sort of gaps or where calls will come in or", "start": month and um the majority of folks that are not in health", "start": 4099.92, "end": 4105.679}, {"text": "or in the emergency services that uh do CPR it's usually on a loved one or a family member so it's a a little uh PSA", "start": 4105.679, "end": 4112.758}, {"text": "is definitely um the more people in your community that um know first aid and have the ability to do CPR and the and", "start": good indicator of community cardiac out of Hospital Cardiac Arrest health so", "start": 4125.359, "end": 4131.08}, {"text": "it's it's a it's a very good um uh PSA to take forward for sure well thank response system there's two levels of response one of them is light and siren one of", "start": 4158.96, "end": 4164.758}, {"text": "them isn't the new one has five and it's color based so purple is someone not breathing sounds like someone's in", "start": 4164.759, "end": 4171.12}, {"text": "Cardiac Arrest red is um a chest Bain um very urgent still licens sir in response", "start": 4171.12, "end": 4178}, {"text": "um for those cases it is whatever the closest available resources okay it", "start": 4178, "end": 4183.278}, {"text": "might be we have one at the hospital it might be we have one on on the road dispatch will assign to whatever the", "start": 4183.279, "end": 4188.92}, {"text": "closest available resource is uh for the lower level priorities um and this is", "start": 4188.92, "end": 4194.52}, {"text": "what it's it's a benefit and we talk about uh burnout of our Workforce so what what would happen now uh you could", "start": 4194.52, "end": 4199.719}, {"text": "have a paramedic that is say they've worked 10 hours in a row they're finally eating something and then they get a", "start": 4199.719, "end": 4205.8}, {"text": "call for someone with a headache when they arrive their headache is actually worse than the than the person's so in the new system those cases may be", "start": 4205.8, "end": 4212.84}, {"text": "prioritized less and what we use as a a best unit response meaning that it takes into account uh workload it takes into", "start": 4212.84, "end": 4218.96}, {"text": "account the meal rest periods and then we respond with the most appropriate ambulance to to that so um uh and if", "start": 4218.96, "end": 4226.239}, {"text": "there's a a vehicle that's on the way to a lower priority and they're the closest to a higher priority we move the the", "start": 4226.239, "end": 4231.64}, {"text": "resource to make sure that those highest priority calls get the highest priority response okay uh final question so uh I", "start": 4231.64, "end": 4238.32}, {"text": "know that in the past there's been some uh concern and controversy maybe over", "start": 4238.32, "end": 4244}, {"text": "how long paramedics are required to stay at the hospital once they've offloaded a", "start": 4244, "end": 4249.08}, {"text": "patient I I I've had a hard time understanding why they would need to still be there and maybe you could", "start": 4249.08, "end": 4254.56}, {"text": "explain that I mean once they've offloaded the patient they're in the doctor's hands I'm not sure what the Pat", "start": 4254.56, "end": 4260.08}, {"text": "the the paramedics add to that equation and I I I'm it's a question because I don't know the answer um but also uh", "start": 4260.08, "end": 4267.88}, {"text": "what's being done about that excellent excellent question and I could probably", "start": 4267.88, "end": 4273.08}, {"text": "talk for hours about this one as well so for uh for you Mr Mayor um so what what you're referring to is ambulance offload", "start": 4273.08, "end": 4279.04}, {"text": "time and uh historically um if we go back to about the mid 2000s 2005 6 78", "start": 4279.04, "end": 4286.199}, {"text": "this was a a huge challenge for us and so part of it is paramedics would are responsible for uh care for the", "start": 4286.199, "end": 4293.6}, {"text": "individual that's in their u in their sort of chain of chain of care and it's not until they transfer that care over", "start": 4293.6, "end": 4299.48}, {"text": "to the hospital that they're able to leave um a number of years ago this was a significant issue hours and hours um", "start": 4299.48, "end": 4306.159}, {"text": "because the there was no beds that were available for the the the nursing staff at the hospitals um to take um over", "start": 4306.159, "end": 4314}, {"text": "responsibility for care for those those patients uh we worked a lot with our local hospitals um with the province and", "start": 4314, "end": 4320.6}, {"text": "we there was a number of Investments uh there was a uh memorandum of understanding from the region that essentially was a pay for performance", "start": 4320.6, "end": 4327.32}, {"text": "for an additional Capital stream for the hospitals that's that's no longer in place but what that allowed us to do is", "start": 4327.32, "end": 4333.159}, {"text": "um uh the hospitals that were underperforming we were able to hold back some of those Capital funds we invested that into Improvement", "start": 4333.159, "end": 4340}, {"text": "initiatives you've seen some responsiveness from the provincial government in terms of our dedicated offn offload nurse funding which is a", "start": 4340, "end": 4347.719}, {"text": "separate stream which we receive about $2.7 million a year from the provincial government um to um provide to the", "start": 4347.719, "end": 4355.6}, {"text": "hospitals for them to provide nursing staff and so what that allows us to do now is uh for the paramedics to bring", "start": 4355.6, "end": 4361.76}, {"text": "the patients in they're actually able to transfer that uh care over to the hospital and then leave much quicker and", "start": 4361.76, "end": 4367.6}, {"text": "so the regional hospitals um have some of the best times in the in the province some of that is the um uh Investments a", "start": 4367.6, "end": 4374.76}, {"text": "lot of that is the leadership at the hospitals and being at the table and really engaging as a collective that is", "start": 4374.76, "end": 4379.8}, {"text": "really important for our community to have paramedics in the 911 system not sitting at the hospital not able to", "start": 4379.8, "end": 4386.12}, {"text": "respond okay thanks a lot I this has uh been a oh go ahead thank you through you Mr chair um", "start": 4386.12, "end": 4394.679}, {"text": "again thank you for the for this chance to discuss and and I wasn't in I was at no time trying to say you shouldn't call", "start": 4394.679, "end": 4400.76}, {"text": "an ambulance if you think you need one but um I had occasion to call one and I'm not blaming anybody but it took 20", "start": 4400.76, "end": 4407.4}, {"text": "minutes to get there and I all I'm saying is I don't think the public necessarily evaluates when to call an", "start": 4407.4, "end": 4413.44}, {"text": "ambulance or not so it is an education piece right because obviously the ambulance people were doing something", "start": 4413.44, "end": 4419.719}, {"text": "else when they couldn't come and and end up being great and fine and they were wonderful that was my only point is that", "start": 4419.719, "end": 4425.56}, {"text": "educating the public and when do you call an ambulance my other question is and you don't have to answer this but is", "start": 4425.56, "end": 4431.08}, {"text": "there any correlation between the millions of people who have family doctors and the number of calls you're", "start": 4431.08, "end": 4437.48}, {"text": "starting to get of stuff that perhaps isn't urgent uh through you Mr chair I can't answer that yes there's a there's", "start": 4437.48, "end": 4444.8}, {"text": "a a significant driver um lack of access to Primary Care um means that uh the", "start": 4444.8, "end": 4451.32}, {"text": "community then goes to what is open and what is available 911 is 24 hours a day the hospital is 24 hours a day it", "start": 4451.32, "end": 4458.12}, {"text": "absolutely is um it is a capacity issue and it it does Drive folks to seek um", "start": 4458.12, "end": 4463.56}, {"text": "alternative um access to Medical Care Mr chair thank you so um the fact", "start": 4463.56, "end": 4470.04}, {"text": "that somebody has no the walk-in is closed they don't have a family doctor they call you and how often would you", "start": 4470.04, "end": 4475.56}, {"text": "say that happens does it happen daily I I don't I don't have those figures but um I could certainly um provide those to", "start": 4475.56, "end": 4483.08}, {"text": "you what I what I can say is through through you Mr chair is that um our highest priority calls people not", "start": 4483.08, "end": 4489.6}, {"text": "breathing um people that don't have a pulse it represents about 2% of our cases what I can say is um those that", "start": 4489.6, "end": 4497.04}, {"text": "have like chest pain stroke like really significant medical Emer issues represents around 20 to 25% of our case", "start": 4497.04, "end": 4504.8}, {"text": "load and the bulk is for um less urgent instances they could still be emergent", "start": 4504.8, "end": 4510.36}, {"text": "they could still require um ambulance intervention but there is certainly a percentage of our our cases that are um", "start": 4510.36, "end": 4517.28}, {"text": "very low uh priority from how we um measure Acuity with with patients which", "start": 4517.28, "end": 4522.36}, {"text": "is a a standardized tool called the Seas or the Canadian triage and acut scale which is the same as the", "start": 4522.36, "end": 4527.96}, {"text": "hospitals you um you Mr chair so um I saw your plan to you know to get more", "start": 4527.96, "end": 4533.84}, {"text": "ambulances everything at this moment in time do you feel that that York Region is staffed appropriately with ambulances", "start": 4533.84, "end": 4540.96}, {"text": "and staff through through you Mr chair um within the within yes the answer is", "start": 4540.96, "end": 4547.08}, {"text": "yes um we've we've done an incredible job and to be honest um our model and um", "start": 4547.08, "end": 4552.48}, {"text": "is what the rest of the 50 something services in the province are seeking to to have so we have a very well uh", "start": 4552.48, "end": 4559.6}, {"text": "staffed well um uh developed infrastructure and it's because of 20", "start": 4559.6, "end": 4564.679}, {"text": "plus years of of really good U Master planning and trying to project uh demand", "start": 4564.679, "end": 4569.84}, {"text": "and trying to meet that with the sort of supply of resource so we have a very good system okay thank you thank you for", "start": 4569.84, "end": 4575.4}, {"text": "all you
